Setup AppVeyor to build from GitHub and deploy to Azure

Recently I set up ApVeyor CI for a new project to build the source code from GitHub, securely connect to Azure SQL database to run tests, and then deploy to Azure. I have written a post to share the end-to-end configurations in detail:
